Job summary

21st Century Home Health Services is seeking a licensed Physical Therapist Assistant to provide in-home therapy across Northern California. You will assess patients, design treatment plans, and promote mobility while delivering high-quality care in patients’ homes.

The role offers flexible scheduling, autonomy, and support with CEU opportunities, mileage reimbursement, and a device for charting and communication. Experience with high-acuity patients is a plus.

Qualifications

• Requires California PTA license and active CPR certification.
• Home health experience is highly preferred.
• Proficient in basic computer use and electronic charting.

Responsibilities

• Examine each patient and develop a plan using treatment techniques to promote movement, reduce pain, restore function, and prevent disability.
• Work with patients to prevent loss of mobility by creating fitness- and wellness-oriented programs.

• License PTA in California
• Home Health experience is highly preferred
• Current CPR License
