Job summary

UPLIFT HOMECARE, LLC is seeking a Licensed Physical Therapist Assistant (PTA) to join our home health team serving Odessa, FL and surrounding areas. This part-time position offers one-on-one patient care in patients' homes with a supportive clinical team focused on independence, mobility and quality of life.

Compensation is up to $60 per hour based on experience and qualifications, with flexible scheduling to support work-life balance.

Qualifications

• Current Florida PTA license.
• Graduate of an accredited PTA program.
• Minimum of 1 year of home health experience.
• Current CPR Certification.
• Valid driver's license and reliable transportation.
• Ability to travel to Odessa and surrounding areas.

Responsibilities

• Provide therapeutic exercises and interventions under the direction of the supervising Physical Therapist.
• Assist patients in improving mobility, strength, balance, and functional independence.
• Observe, document, and report patient responses and changes in condition.
• Assist with implementing and maintaining individualized treatment plans.
• Complete clinical documentation accurately and within required timeframes.
• Educate patients and caregivers on exercises, mobility, safety, and treatment goals.
• Communicate effectively with the supervising Physical Therapist and interdisciplinary care team.
• Participate in care coordination and quality improvement activities.
• Maintain current knowledge of physical therapy practices and standards.
