Qualifications

Active Physical Therapist Assistant license in New Jersey (or eligible to obtain).
New grads welcome; we provide mentorship and training.
Experience in SNF, long-term care, or geriatrics is a plus.
Strong communication and interpersonal skills.
Ability to work collaboratively as part of an interdisciplinary team.
Compassionate, positive attitude with a genuine desire to help residents.

Responsibilities

Implement treatment plans developed by the Physical Therapist to improve residents' strength, balance, gait, and mobility.
Treat residents with orthopedic, neurological, or post-surgical conditions under supervision of a Physical Therapist.
Collaborate with physicians, nurses, occupational and speech therapists.
Document patient progress accurately in the EMR in a timely manner.
Participate in care plan meetings and family conferences.
Provide compassionate, patient-centered care in a team environment.
Report to the Physical Therapist and Director of Rehabilitation.
