Job summary

Health, Inc. seeks a Physical Therapy Assistant (PTA) to deliver patient-centered therapy under the supervision of a licensed physical therapist in a home health setting.

You will provide direct PT services, document progress, educate patients and caregivers, and collaborate with the care team to support discharge planning. Strong organizational and communication skills with an active PTA license and CPR certification are required.

Qualifications

• Graduation from an accredited Physical Therapist Assistant program.
• Current, unrestricted state license and CPR certification.
• Strong organization, communication, attention to detail, and time management skills.
• Experience in home health therapy and OASIS documentation is preferred.

Responsibilities

• Provide direct physical therapy services, treatments, and procedures to patients.
• Communicate regularly with patients and caregivers about the plan of care.
• Observe and report patient conditions, reactions to treatments, and any changes to the physician or RN.
• Assist in relieving pain and restoring or maintaining function to optimize performance.
• Educate patients and caregivers on safe use of assistive devices and rehabilitation techniques.
• Collaborate with interdisciplinary team members and plan for patient discharge.
• Ensure documentation of clinical notes, progress, and daily reports for continuity of care.
• Maintain confidentiality, adhere to ethical standards, and comply with professional conduct guidelines.
• Participate in in-service programs and perform other related duties as assigned.
