Northshore Home Health Care in Front Royal, VA seeks a Physical Therapy Assistant (PTA) to provide direct patient care under the supervision of a licensed physical therapist, following the physician's plan of care.

You will assess and treat patients, educate families, document progress, and coordinate with an interdisciplinary team, with emphasis on compassionate, compliant home health therapy and keeping patient safety at the forefront.

Qualifications

• Graduation from an accredited PTA program.
• Current, unrestricted state licensure as a PTA.
• Valid CPR certification.
• Preferred experience in home health therapy and OASIS documentation.

Responsibilities

• Assess, treat, and educate patients to improve or restore function and relieve pain.
• Communicate regularly with patients and caregivers about the care plan.
• Provide skilled therapy services and ensure patient safety during treatments.
• Report patient progress and any changes to supervisors and physicians.
• Educate patients and families on the use of prosthetic, orthotic, and assistive devices.
• Collaborate with interdisciplinary team members on patient care plans.
• Plan for discharge, prepare instructions, and communicate discharge details.
• Maintain documentation, records, and ensure confidentiality of patient information.
• Participate in ongoing education and adhere to professional standards.
• React productively to change and perform additional assigned duties.
