Job summary

Greenfield Rehabilitation Agency in Union Grove, WI is seeking a PRN Physical Therapist Assistant to support our diverse patient population across skilled nursing, senior living, and in‑home care settings.

As an PTA, you will implement therapy interventions under a Physical Therapist's supervision, help residents improve mobility and independence, and document progress to ensure high-quality care in a compassionate, team-based environment.

Qualifications

• Graduate of an accredited school with an associate degree as Physical Therapy Assistant.
• Certification by the State of Wisconsin to practice as a Physical Therapy Assistant.

Responsibilities

• Assist in implementing physical therapy interventions under the supervision of a Physical Therapist.
• Work with residents to improve mobility, strength, and physical function.
• Help residents recover from injuries, surgeries, or illnesses by providing physical therapy exercises and techniques.
• Support residents in improving balance, coordination, and functional mobility to regain independence.
• Monitor and report progress to the supervising Physical Therapist and adjust treatment plans as needed.
• Educate residents and their families on mobility strategies, injury prevention, and rehabilitation exercises.
• Collaborate with the multidisciplinary care team to ensure holistic care.
• Participate in care planning meetings to discuss resident progress and adjust therapy goals.
• Maintain accurate records of treatments, progress, and documentation to meet healthcare regulations.
• Ensure residents receive therapy in a compassionate, safe, and supportive environment.
