Greenfield Rehab is seeking a Physical Therapist Assistant to join our therapy team serving senior communities, skilled nursing facilities and home health across Wisconsin. You will deliver high-quality, patient-centered care across the continuum of care and collaborate with an interdisciplinary team to support transitions of care.

You will practice at the top of your license, provide meaningful therapy, and contribute to a strong team culture with leadership that values your expertise.

Qualifications

• Graduate from an accredited school with an associate degree as a Physical Therapist Assistant.
• Licensed by the State of Wisconsin to practice as a Physical Therapist Assistant.

Responsibilities

• Provide meaningful, patient-centered therapy across multiple levels of care.
• Collaborate with interdisciplinary team to support transitions of care and quality outcomes.
