Job summary

Caringhand Home Health Solutions, Inc is seeking a Physical Therapist Assistant to work per diem delivering in-home physical therapy under a licensed PT's supervision.

Responsibilities include guiding patients through exercises, transfers, and device use; applying modalities; and documenting care while communicating progress to the care team. Travel to patient homes is required, with potential for full-time conversion based on performance.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active PTA license from the Physical Therapy Board of California.
• Current CPR certification with AHA.
• Strong communication and organizational skills.
• Valid driver's license and willingness to travel to patient homes.

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and use of assistive devices.
• Apply modalities such as heat, cold, ultrasound, electrical stimulation, and massage.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, participate in meetings and ongoing training.
• Perform other duties as required for patient services

As a Physical Therapist Assistant with Caringhand Home Health Solutions, you will work alongside a licensed Physical Therapist to help patients recover, regain mobility, and return to the activities they love — all from the comfort of their homes.

Please note that this role is per diem with a rate of $90 - $95 per visit.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices such as wheelchairs, braces, crutches, canes, and prosthetics.
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
