Job summary

Caring Hand Home Health Solutions, Inc. is seeking a licensed Physical Therapist Assistant to work alongside a supervising Physical Therapist to deliver in-home rehabilitation. You will guide patients through therapeutic exercises, gait training, and use of assistive devices, helping them regain mobility.

This per diem role involves travel to patient homes, documentation, and collaboration with care teams. Requirements include an Associate's degree from an APTA-accredited PTA program, an active

• Associate's degree from an APTA-accredited Physical Therapy Assistant program.
• Active PTA license from the Physical Therapy Board of California.
• Current CPR certification with AHA.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices such as wheelchairs, braces, crutches, canes, and prosthetics.
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services

As a Physical Therapist Assistant with Caring Hand Home Health Solutions, you will work alongside a licensed Physical Therapist to help patients recover, regain mobility, and return to the activities they love — all from the comfort of their homes.

Please note that this role is per diem with a rate of $90 - $95 per visit.
