Concierge Home Care is seeking a dedicated Physical Therapist Assistant to deliver in-home therapy services to patients in their homes across Tallahassee. You will follow the PT’s treatment plan, educate patients and caregivers, and collaborate with an interdisciplinary team to support recovery in a comfortable home setting.

Qualifications

• Active, unencumbered Florida Physical Therapist Assistant License (Required).
• Valid driver's license, reliable transportation, and auto insurance (Required).
• CPR Certification (Preferred).

Responsibilities

• Implement in-home therapy services under the supervision of a licensed Physical Therapist.
• Follow individualized treatment plans based on the PT’s initial evaluation.
• Educate patients and caregivers on specific exercises and safety techniques.
• Collaborate and communicate patient progress with the interdisciplinary care team.
