Job summary

Elderwood at North Creek is seeking a dedicated Physical Therapist Assistant to join our rehab team in a full-time capacity. You will collaborate with a supervising Physical Therapist to implement treatment plans, monitor patient progress, and document outcomes to support residents’ mobility and independence.

The role emphasizes compassionate, team-oriented care, with opportunities for growth and continuing education within a supportive interdisciplinary setting in the Adirondacks.

Qualifications

  • Graduation from a PTA program required.
  • New York State license to practice required.
  • Current CPR/BLS credentials.

Responsibilities

  • Assist the Physical Therapist in planning, developing and carrying out treatment programs and procedures for residents.
  • Write observations of resident’s behavior and progress in Progress Notes.
  • Maintain statistical records and other departmental reports as requested by Physical Therapist.
  • Assist with in-service training programs upon request.
  • Assist with keeping supplies in good order and daily sanitation of equipment.
  • Arrive to work on time, regularly, and as scheduled; responsible for own transportation.
  • Utilizes electronic timekeeping system.
  • Observe and practice confidentiality of resident information and privacy when performing job duties.
  • Recognize and follow the dress code of the facility including wearing name tag at all times.
  • Perform assigned duties according to service excellence standards and goals.

Full-Time Physical Therapist Assistant (PTA)

Physical Therapist Assistant team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments as developed and recommended by a Registered Physical Therapist, ensuring that the highest degree of quality care is maintained at all times.

  • Working one-on-one with residents to improve mobility, strength, balance, and endurance
  • Assisting patients through individualized therapy treatment plans created by the supervising PT

Qualifications

In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
