You will manage your own caseload, perform evaluations, reassessments, and develop plans of care while collaborating with physicians and an interdisciplinary team in a supportive environment.

Qualifications

• California PTA license required or eligible.
• Graduated from CAPTE-accredited PTA program.
• Passed NPTE exam for PTAs.
• Valid CA driver’s license with clean record.
• New to home health? Training available.

Responsibilities

• Perform initial evaluations and develop plans of care.
• Reassess at least every 30 days or as required.
• Deliver physician-ordered skilled PT services.
• Educate patients and caregivers to promote independence.
• Complete OASIS and clinical notes within timeframes.
• Communicate with physicians about progress and changes.
• Participate in care conferences and QAPI activities.
• Collaborate with interdisciplinary team for coordinated care.

Per Diem or Full-Time Physical Therapist Assistant (Home Health) – Marin County, CA

Location: Marin County and surrounding areas

Pay: From $90.00 per hour + mileage reimbursement

Provide skilled physical therapy services to homebound patients in their homes throughout Marin County under Physical Therapist supervision.

Daily Responsibilities

• Implement PT-designed treatment plans for patients
• Therapeutic exercises, gait training, range of motion, transfer training
• Patient/family education on home exercise programs and safety
• Collaborate with PT, OT, RN, and physician teams
• Work with post-surgical, stroke recovery, orthopedic, and geriatric patients

Territory

Marin County primarily, with Sonoma and San Francisco Counties as needed

Required Qualifications

• California PTA license (or eligible - we can hire while processing!)
• Graduated from CAPTE-accredited PTA program
• Passed NPTE exam for PTAs
• Valid CA driver's license with clean record

Preferred (We'll Train You!)

• Home health experience not required
• 1+ year healthcare experience
• Geriatric population experience

• Complete reassessments at least every 30 days or as required
• Deliver physician-ordered skilled PT services
• Provide patient and caregiver education to promote independence and safety
• Accurately complete OASIS documentation and clinical notes within agency timeframes
• Communicate effectively with physicians regarding progress and changes in condition
• Participate in care conferences and QAPI activities
• Collaborate with interdisciplinary team members to ensure coordinated care

Qualifications

• Graduate of a CAPTE-accredited Physical Therapy program
• Current California PT license in good standing
• Minimum of 1 year of clinical experience preferred (home health experience a plus)
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently and exercise sound clinical judgment
• Strong documentation and communication skills
