Job summary

Best Home Health Providers Inc. is hiring a Physical Therapist Assistant to work with a licensed Physical Therapist in patients’ homes across the Bay Area and beyond. The role is per diem with compensation of $95 per visit, and offers flexible scheduling, with potential to convert to full-time based on performance.

You’ll help patients recover mobility and return to activities they love, while traveling to patient homes and delivering high-quality care.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active PTA license from the California Physical Therapy Board.
• Current CPR certification with AHA.
• Strong communication and organizational skills.
• Valid driver’s license and insured vehicle; willing to travel to patient homes.

Responsibilities

• Deliver hands-on physical therapy under a licensed PT and follow the care plan.
• Guide patients through exercises, gait and transfer training, and assistive devices.
• Apply modalities (heat/cold/ultrasound/electrical stimulation/massage) to relieve pain and restore function.
• Monitor progress and communicate changes to the PT and care team.
• Document care accurately and timely; participate in team meetings and training.
• Perform other duties as required for patient services.

As a Physical Therapist Assistant with Best Home Health Providers, you will work alongside a licensed Physical Therapist to help patients recover, regain mobility, and return to the activities they love — all from the comfort of their homes.

Please note that this role is per diem with a rate of $95 per visit.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices such as wheelchairs, braces, crutches, canes, and prosthetics.
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
