Job summary

Premier Home Health Providers Inc. is seeking a per diem Physical Therapist Assistant to support patients in their homes, working alongside a licensed Physical Therapist to promote mobility and independence.

The role involves hands-on therapy, gait training, and use of assistive devices, with the aim of preventing rehospitalizations and improving quality of life in the comfort of patients' own homes. Travel to patient homes is required; a valid CA license and CPR certification are essential.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active California PTA license.
• Current CPR certification with AHA.
• Valid driver's license with reliable insured vehicle and willingness to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices.
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
