Job summary

Best Home Health Providers Inc. in California is seeking a per diem Physical Therapist to support patients at home, delivering skilled therapy and helping mobility and independence.

You will evaluate functional status, implement plans of care, educate families, and collaborate with physicians while supervising assistants as needed. A CA license, degree in PT, CPR, and a valid driver’s license are required; the role offers flexible scheduling and per-visit compensation.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills with patients and families.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $160 - $180 per visit.
