AgeRight Care at Home in Portland, OR is seeking a Physical Therapist Assistant to join our care-at-home team on the Eastside. You will carry out care plans, document progress notes, and educate patients and families to meet individualized goals.

Eligible candidates have an accredited PTA degree, Oregon license or in process, CPR, and NPI on hire; home health experience is preferred.

Qualifications

• Graduated from an accredited Physical Therapist Assistant program.
• Current state licensure or in process to obtain licensure.
• CPR Certification is required.
• National Provider Identifier (NPI) required upon hire.

Responsibilities

• Carry out tasks from the care plan.
