TruCare Living Center in Palestine, TX, is seeking a full-time Physical Therapist Assistant to join our in-house rehab program. The role supports a geriatric population in skilled nursing and post-acute care settings.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care with plans aligned to standards.
• Provide rehabilitative, skilled, and medically necessary treatments.
• Assist with patient transfers, standing, and mobility as needed.
• Perform rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Counsel patients, families, and care teams on physical disorders and care plans.
• Generate discharge summaries compliant with regulatory requirements.

TruCare Living Center of Palestine, TX, is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist Assistant for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist Assistant license is required.
• Open to PTAs at all experience levels. New Grads are welcome!
