Flagship Therapy’s Fairfield Post Acute Care in Fairfield, CA, is seeking a dedicated PRN Physical Therapist Assistant to join our in-house therapy team and help expand our programs, including wound care and cognition initiatives.

As a PTA, you’ll screen and treat patients, develop care plans, document discharge summaries, and collaborate with families and other clinicians in a supportive clinical setting. Pay is $53/hour based on experience, with a flexible PRN schedule.

Qualifications

• Occupational Therapy Assistant license is required.
• Open to Occupational Therapy Assistant at all experience levels. New Grads are welcome!

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Assist with standing, walking, lifting, transferring, and repositioning of patients and equipment.
• Administer procedures as part of the rehabilitation plan; manual techniques, ambulation, therapeutic exercises, modalities, wound care.
• Provide consultation to patients, families, caregivers, and other service providers.
• Generate discharge summaries of services provided in compliance with regulations.

Education

Occupational Therapy Assistant license is required.

PHYSICAL THERAPIST ASSISTANT - PRN

Fairfield Post Acute Care in Fairfield, CA, is currently seeking a PRN Physical Therapist Assistant to join our crew of in-house therapists and help us expand therapy programming to include Abilities Care (cognition), ACP programs, Big and Loud and wound care. As a therapy professional, you will have the freedom to create and implement additional programs to best meet the needs of the patients in this facility.

Pay: $53/hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
