Job summary

Remedy Home Health Care in East LA is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team. You will deliver skilled in-home physical therapy under a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility for seniors.

Qualifications

• Active California PTA license required.
• Current CPR certification (AHA required).
• Minimum 1 year of clinical experience, home health preferred.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises, mobility training, and rehab programs.
• Support senior care patients in improving strength, balance, and independence.
• Monitor, document, and report patient progress to the supervising PT.
• Educate patients and caregivers on home exercise programs, safety, and fall prevention.
• Collaborate with physicians, RNs, and the interdisciplinary care team.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
