Evergreen Physical Therapy is seeking a dedicated Physical Therapist Assistant to join our Michigan-based clinics. You will review referrals, deliver direct patient care, and document progress per established guidelines across multiple locations.

The role emphasizes collaboration with physicians, insurance providers, and care teams to support patient recovery. The ideal candidate is a PTA graduate with MI licensure and CPR certification, capable of autonomous work and eager to engage in balance

Qualifications

• Graduates from an accredited Physical Therapy Assistant program.
• Current state licensure as a PTA in Michigan and CPR certification.
• Bilingual English and Spanish is a strong plus.

Responsibilities

• Review the physician's referral and patient records to determine treatment.
• Provide direct patient care per established protocols and guidelines.
• Document findings, progress, and instructions for patients and caregivers.
• Implement outpatient PT treatment plans and provide essential therapies.
• Communicate with patients, caregivers, physicians, and insurance companies.
• Ensure timely documentation including evaluations, progress notes, and discharge summaries.

We are seeking a passionate and dedicated Physical Therapist Assistant to join our team at Evergreen PT.

As a key member of our organization, you will play a crucial role in delivering high-quality care to our patients, ensuring their recovery and well-being.

• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate degree from accredited Physical Therapy Assistantprogram.New graduates are encouraged to apply, we'd love for you to start your career with us!
• Current state of Michigan license and CPR certification
• Ability to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans or willingness to learn
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office,
• Bilingual (English and Spanish) is a strong plus.
