Evergreen Physical Therapy in Michigan seeks a passionate Physical Therapist Assistant to join our team. You will deliver high-quality patient care, review referrals, and help patients recover across our five clinic locations.

The role requires a Michigan PTA license, CPR certification, and ability to implement balance and vestibular rehab protocols. Bilingual English/Spanish is a strong plus, and you will work in an autonomous environment with strong team support and ongoing professional

Qualifications

• Graduate of an accredited PTA program.
• Current Michigan PTA license and CPR certification.
• Willingness to learn balance and vestibular rehab protocols.
• Ability to work autonomously and efficiently.
• Proficient with EMR, database software, and MS Office.
• Bilingual English/Spanish is a plus.

Responsibilities

• Review referrals and patient records to determine PT treatment.
• Provide direct patient care per established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Implement physical therapy treatment programs and essential treatments.
• Communicate effectively with patients, caregivers, physicians and insurers.
• Ensure timely documentation including daily notes and reports.

Job Description

We are seeking a passionate and dedicated Physical Therapist Assistant to join our team at Evergreen PT.

As a key member of our organization, you will play a crucial role in delivering high-quality care to our patients, ensuring their recovery and well-being.

• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

Qualifications

• Graduate degree from accredited Physical Therapy Assistant program. New graduates are encouraged to apply, we’d love for you to start your career with us!
• Current state of Michigan license and CPR certification
• Ability to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans or willingness to learn
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office,
• Bilingual (English and Spanish) is a strong plus.
