Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for a full-time opportunity to provide senior living visits in the Central Bucks County area.

The role includes delivering home-based PT, wound care, and independent living support, with a $10,000 sign-on bonus for full-time service. Responsibilities include home visits, mobility assessments, plan development, and client family education, all while maintaining thorough EMR documentation and adherence to BAYADA

Qualifications

• PA Physical Therapist license
• Minimum of one year of PT experience
• Graduate from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement protective interventions for wounds and promote wound repair with evidence-based dressings.
• Develop and implement individualized care plans including exercises, gait training, and balance training.
• Educate clients, family members, or representatives in rehabilitative care and activities for health and independence.
• Document observations, interventions, and evaluations in the EMR.
