The position offers a $10,000 sign-on bonus and a negotiable salary between $100,000 and $130,000 per year. Travel within a 40-mile radius of Lorain, OH is required to visit patients at homes or facilities.

Qualifications

• Current Physical Therapist Licensure
• Bachelor’s degree in Physical Therapy
• Ability to travel to patients’ homes or facilities
• Home healthcare experience preferred
• Daily commute within 40-mile radius of Lorain, OH

Responsibilities

• Supervision to Asst. therapy staff
• Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll
• Keeping open communication with patients and home health agencies to promote a positive and caring experience
• Facilitating communication with Physical Therapist Assistant to manage patient care
• Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork
• Attend meetings and case conferences
