Job summary

Rivetus is seeking a licensed Physical Therapist to provide high-quality care to geriatric post-acute patients at our Whitehall facility. You will assess, plan and implement therapies to restore physical function and independence.

Responsibilities include coordinating with PTAs, documenting care, participating in care conferences, and pursuing ongoing professional development to maintain current practice standards. Michigan licensure is required.

Qualifications

• Completion of an accredited Physical Therapy program.
• Licensure as required by the State of Michigan.
• Strong clinical judgment and ability to plan patient care.”
• Ability to coordinate with PTAs and other staff.

Responsibilities

• Conduct physical therapy screenings and evaluations to determine patient physical status.
• Provide physical therapy treatment in line with APTA standards and best practices.
• Develop and implement treatment plans to restore function and independence.
• Supervise PTAs and aides as assigned.
• Document care and maintain high-quality medical records and reimbursement readiness.
• Participate in patient care conferences, CQI activities and continuing education.

Lewis St. Whitehall, MI 49461Schedule: Full Time, Monday-Friday (Schedule Flexibility Available!)General Summary:In this role you will provide physical therapy services to patients impaired by physical dysfunction, including assessment, treatment planning and implementation of treatment plans. Participates in patient care conferences, program development, and screenings as appropriate. Provides supervision for PTAs and aides as assigned.
