Job summary

AdvisaCare is seeking a licensed Physical Therapist to provide patient-centered home health services in Michigan. You will evaluate, plan, and implement personalized treatment to restore movement and function, working one-on-one with clients in their homes.

Responsibilities

• Develop and implement individualized treatment plans focused on mobility, strength, balance, and function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.

Requirements

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan
• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!

• Compassionate Caregiver - a genuine desire to help others and promote their independence is vital
• Self-Starter - the ability to work autonomously, manage your own schedule, and track patient progress is key
• Transportation - a valid driver's license and reliable vehicle for travel to patient locations
