Job summary

AdvisaCare in Michigan is seeking a licensed Physical Therapist (PT) to join our patient-centered team in Traverse City. You will evaluate patients, develop customized treatment plans, and guide mobility, strength, and balance improvements in home health settings.

This role emphasizes compassionate, patient‑focused care and collaboration with an interdisciplinary team. Requirements include: Michigan PT license, reliable transportation, and strong communication with patients and families;

Qualifications

• Licensed Physical Therapist in Michigan.
• Minimum 1 year Home Health care preferred.
• Team player with strong communication.
• Autonomous worker with scheduling flexibility.
• Valid driver's license and reliable vehicle.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

Requirements

What We're Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan
• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!
