Job summary

AdvisaCare is hiring a Licensed Physical Therapist in Michigan to provide patient-centered care in home health settings. You will develop individualized treatment plans, monitor progress, and educate patients and families while collaborating with a multidisciplinary team.

The role emphasizes compassionate caregiving, autonomy in managing your schedule, and a commitment to improving mobility and safety for clients in their communities.

Qualifications

• Licensed Physical Therapist with Michigan license.
• Minimum of 1 year in home health care preferred.
• Strong communication and interpersonal skills.
• Ability to work autonomously and manage your schedule.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance and function.
• Conduct therapeutic exercises and interventions to restore independence and reduce pain.
• Assess and document patient progress; adjust plans as needed.
• Collaborate with interdisciplinary teams to ensure continuity of care.
• Educate patients and families on exercises, safety, and injury prevention.
• Maintain accurate documentation and adhere to professional standards.

Responsibilities

• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

What We’re Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan.
