Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a licensed Physical Therapist to deliver in-home and facility-based services across our service area. You will assess patients, develop personalized care plans, and help restore mobility, balance, gait, and independence in the comfort of patients' homes.

Qualifications

• Active Massachusetts Physical Therapist license.
• Graduate of an accredited PT program (DPT or MPT).
• CPR certification.
• Minimum 1 year of PT experience preferred (new grads welcome with supervision).

Responsibilities

• Perform in-home PT evaluations and develop personalized care plans.
• Deliver skilled therapy to improve strength, balance, gait, mobility.
• Educate patients and families on home exercise and safe mobility.
• Collaborate with BrightStar team including nurses, OTs, HHAs, and case managers.
• Document patient progress and visit notes using mobile EMR.
• Ensure patient safety and outcomes aligned with BrightStar standards.

Requirements

• Active and unrestricted Massachusetts Physical Therapist license
• Graduate of an accredited PT program (DPT or MPT)
• CPR certification
• Minimum 1 year of PT experience preferred (new grads welcome with supervision)
• Reliable vehicle and valid driver's license
• Ability to work independently and as part of a care team
