Sentara Health is seeking a licensed Physical Therapist in Stafford, VA to assess, plan, organize, and implement rehabilitative programs that improve mobility, relieve pain, and address disabling conditions from disease or injury. You will provide education to patients and caregivers and supervise care provided by team members.

• Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
• Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
• Establishes and updates formal plan of care to address individual patient needs.

Productive and efficient in daily operations. Selects and performs the appropriate assessment procedures. Establishes and updates formal plan of care to address individual patient needs.
