Emplify Health, a partnership of Bellin Health and Gundersen Health System, seeks a full-time Physical Therapist to join our compassionate care team in Wisconsin. You will collaborate with patients to assess needs, develop multidisciplinary plans, and document progress from initial evaluation through discharge.

Qualifications

• Graduate from an accredited program for Physical Therapy.
• Current licensure as a Physical Therapist in the State of WI.
• CPR Certification (or ability to obtain within 3 months).

Responsibilities

• Work closely with patients to assess their physical and psychosocial needs.
• Planning, development, and implementation of multidisciplinary plan of care.
• Evaluation of patients' responses to care.
• Maintain patient records including initial evaluations/assessment, progress notes, and discharge summaries.

Full-time Physical Therapist

What you will do

• Full-time 40 hours/week 8 hour shifts, 7am-3:30pm and 8am-4:30pm; Occasional coverage on weekends
• Work closely with patients to assess their physical and psychosocial needs
• Planning, development, and implementation of multidisciplinary plan of care
• Evaluation of patients' responses to care
• Maintain patient records including initial evaluations/assessment, progress notes, and discharge summaries
