Job summary

Morningstar Residential Care Center in Oswego, NY is seeking a licensed Physical Therapist to provide skilled PT services in a resident-centered setting. The role involves evaluating residents, creating care plans, and collaborating with an interdisciplinary team to restore function and independence.

Qualified applicants hold a NY PT license, a degree from an accredited PT program, and will operate under state and federal regulations.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Bachelor’s Degree required.
• Licensed Physical Therapist in New York State.

Responsibilities

• Provide skilled PT services per physician orders and professional standards.
• Develop individualized plans of care with goals and discharge planning.
• Perform evaluations, annual screens, and nursing-requested assessments.
• Document treatments, progress, and billing per regulations.
• Utilize modalities such as E-stim, ultrasound, and diathermy.
• Supervise Physical Therapy Assistants per regulations.
• Collaborate with physicians, families, and interdisciplinary team members.
• Participate in care meetings, in-services, and ongoing education.
• Maintain patient confidentiality and regulatory compliance.

Pay: Up to $45/hour + $5,000 SIGN ON BONUS!
Schedule: Full Time

• Utilize treatment modalities such as E-stim, ultrasound, and diathermy

• Supervise Physical Therapy Assistants per regulations

• Collaborate with physicians, families, and interdisciplinary team members

• Participate in care meetings, in-services, and ongoing education

• Maintain patient confidentiality and regulatory compliance

You’ll serve as a vital bridge between our rehab department and center staff, always representing our mission with compassion and professionalism.

Qualifications:

• Graduate of an accredited Physical Therapy program

• Bachelor’s Degree required

• Certified and holds a valid Physical Therapy license in New York State
