Job summary

M2 Healthcare is seeking a licensed Physical Therapist to provide resident-centered care in a long-term care setting in New York. You will evaluate residents, plan treatments, select meaningful activities, and monitor progress to promote independence.

Responsibilities include documenting care, auditing records, safeguarding confidentiality, and collaborating with the healthcare team to maintain a safe, supportive environment for residents.

Qualifications

  • Graduate of an accredited Physical Therapy Program.
  • Current New York State PT License.
  • Experience in long term care settings preferred.

  • Maintain documentation of Resident care services by auditing records.
  • Maintains a cooperative relationship among healthcare teams & contributes to a team effort.

Education

Graduate of an accredited Physical Therapy Program
Current New York State PT License
