Job summary

Beachside Post Acute in California is seeking a licensed Physical Therapist for a per diem role. You will evaluate residents, create and implement treatment plans, and provide PT interventions including exercise, manual therapy, and modalities.

Collaboration with an interdisciplinary team and thorough documentation are required. The ideal candidate has a valid California license and at least 2 years in healthcare, preferably in a skilled nursing facility, with knowledge of current PT standards

Qualifications

• Licensed Physical Therapist registered in California.
• Minimum 2 years of healthcare experience, preferably in SNF.
• Knowledge of current physical therapy practice standards and regulations.

Responsibilities

• Evaluate residents' physical abilities, mobility needs, and functional limitations.
• Develop and implement individualized treatment plans.
• Provide skilled PT interventions including exercise, manual therapy, and modalities.
• Monitor progress and adjust plans as needed.
• Educate residents and families on goals and home exercise programs.
• Maintain accurate documentation of services.

Job Summary:

Join our team of compassionate and dedicated healthcare professionals at Beachside Post Acute, a warm and welcoming facility that specializes in compassionate care and rehabilitation. As a Physical Therapist, you will play a critical role in helping our residents achieve their highest level of mobility and independence. We are seeking a licensed Physical Therapist with a passion for providing high-quality care to join our per diem team.

Responsibilities:

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments

• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders

• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities

• Monitor and document resident progress and adjust treatment plans as necessary

• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents

• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs

• Maintain accurate and timely documentation of physical therapy services provided

• Participate in team meetings, in-service education, and quality improvement initiatives

• Maintain current knowledge of physical therapy practice and relevant regulations

Qualifications:

• Current license to practice as a Physical Therapist in the state of California

• Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility

• Knowledge of current physical therapy practice standards and regulations
