IEP Corp seeks a licensed Physical Therapist to join our skilled nursing facility team. You will evaluate and treat residents with mobility impairments to help maximize independence and quality of life.

Qualifications

  • Current California license to practice as a Physical Therapist.
  • Minimum of 2 years of healthcare experience, preferably in a skilled nursing facility.

Responsibilities

  • Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments
  • Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
  • Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
  • Monitor and document resident progress and adjust treatment plans as necessary
  • Communicate with interdisciplinary team members, including physicians, nurses, occupational therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents
  • Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs
  • Maintain accurate and timely documentation of physical therapy services provided
  • Participate in team meetings, in-service education, and quality improvement initiatives
  • Maintain current knowledge of physical therapy practice and relevant regulations
