Job summary

Beachside Post Acute in California is seeking a licensed Physical Therapist to join our per diem team. You will evaluate residents, design customized treatment plans, and provide skilled therapy to improve mobility and independence.

You will document progress, collaborate with an interdisciplinary team, educate residents and families, and stay current with PT standards and regulations while delivering compassionate, patient-centered care.

Qualifications

• Current license to practice as a Physical Therapist in California.
• Minimum of 2 years of healthcare experience, preferably in skilled nursing.
• Knowledge of current PT practice standards and regulations.

Responsibilities

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments.
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities.
• Monitor and document resident progress and adjust treatment plans as necessary.
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents.
• Educate residents and family members on PT goals, treatment plans, and home exercise programs.
• Maintain accurate and timely documentation of PT services provided.
• Participate in team meetings, in-service education, and quality improvement initiatives.
• Maintain current knowledge of PT practice and relevant regulations.
