Job summary

Beachside Post Acute is seeking a licensed Physical Therapist to join our per diem team in California. The role focuses on helping residents achieve mobility and independence through skilled PT interventions and individualized treatment plans.

The ideal candidate will have a California license and at least 2 years in a healthcare setting, preferably SNF, with knowledge of current PT practice standards and regulations.

Qualifications

• Current license to practice as a Physical Therapist in the state of California
• Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
• Knowledge of current physical therapy practice standards and regulations

Responsibilities

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
• Monitor and document resident progress and adjust treatment plans as necessary
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents
• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs
• Maintain accurate and timely documentation of physical therapy services provided
• Participate in team meetings, in-service education, and quality improvement initiatives
• Maintain current knowledge of physical therapy practice and relevant regulations
