Job summary

All Care and Comcare Home Health in California seeks a compassionate Field Physical Therapist to provide skilled therapy in patients' homes with a flexible schedule and a supportive team. You will perform SOC, ROC, Recert, and DC, travel to patient homes, collaborate with the interdisciplinary team, educate patients and families on home exercise programs, and maintain precise documentation.

Qualifications include California PT license, CPR, valid driver's license, and willingness to travel

Position

Field Physical Therapist

Key Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
