All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to deliver skilled PT services in patients’ homes, with a flexible schedule and supportive team environment. You will conduct SOC/ROC/Recert and discharge activities, and collaborate with the care team to meet individualized goals.

Requirements include an active CA PT license, accreditation, strong documentation, and willingness to travel within the service area.

Qualifications

Responsibilities

Field Physical Therapist

Key Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
