Job summary

All Care and Comcare Home Health seeks a compassionate Field Physical Therapist to deliver in-home physical therapy across our service area. You will perform SOC/ROC/recertifications and work closely with families to implement individualized care plans.

The ideal candidate holds a CA PT license, graduates from an accredited PT program, and demonstrates strong documentation and communication skills while traveling to patient homes.

Qualifications

Position

Field Physical Therapist

Key Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
