AdvisaCare is hiring a licensed Physical Therapist in Michigan to provide patient-centered therapy in home health settings. You will evaluate, design and implement individualized treatment plans to improve mobility, strength and independence.

You will educate patients and families, document progress, and collaborate with an interdisciplinary team to ensure safe, effective care. A flexible schedule and supportive workplace are part of AdvisaCare's commitment to its clinicians.

Qualifications

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.

What We’re Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan.
• Experience - while a minimum of one year in home health care is preferred, we’re open to fresh perspectives and new talent!
• Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively.
• Compassionate Caregiver - a genuine desire to help others and promote their independence is vital.
• Self-Starter - the ability to work autonomously, manage your own schedule, and track patient progress is key.
• Transportation - a valid driver's license and reliable vehicle for travel to patient locations.
