Job summary

Amedisys, a leading home health and hospice provider, seeks a Physical Therapist to help patients regain movement and function in their home settings. You will deliver PT interventions, reassess patient progress at least monthly, and coordinate discharge planning with care teams.

Requirements include an active PT license, CAPTE-accredited education, CPR certification, and at least one year in home health (or regional leadership approval for less experience).

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an CAPTE-accredited physical therapy program.
• One year of home health physical therapy experience preferred; if less, mentoring may be required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, adjust plans as needed, and perform discharge planning.
• Other duties as assigned.

• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
