Job summary

Concierge Home Care, based in Tallahassee, FL, seeks a licensed Physical Therapist to deliver patient-centered home health services. You will travel to patients’ homes, perform evaluations, and create goal-oriented treatment plans with input from patients and caregivers.

Innovative technology, including AI-assisted charting, supports documentation and efficiency. You will supervise PT Assistants and collaborate with an interdisciplinary team to ensure quality, compliant care delivery.

Qualifications

• Unencumbered Florida Physical Therapy License (required).
• Valid driver’s license and reliable transportation (required).
• Experience with OASIS documentation (preferred).
• CPR Certification (required).
• Ability to delegate and supervise Physical Therapy Assistants (preferred).

Responsibilities

• Travel to patients’ homes to provide individualized physical therapy services that promote recovery, mobility, and independence.
• Conduct comprehensive evaluations to assess patient conditions and rehabilitation needs.
• Develop and implement goal-oriented treatment plans in collaboration with patients, caregivers, and the interdisciplinary care team.
• Utilize AI-assisted documentation and charting tools to support timely, accurate note completion and faster turnaround times.
• Guide and supervise Physical Therapy Assistants to ensure high-quality, compliant care delivery.

Your Role as a Physical Therapist
