Job summary

Sligo Creek Healthcare is seeking a motivated Physical Therapist to join our rehabilitation team in Maryland. You will evaluate residents, plan therapy, and provide interventions to restore function, reduce pain, and promote independence.

Ideal candidates hold a Maryland PT license with strong assessment, documentation, and communication skills, and are comfortable supervising PTAs within a collaborative, supportive environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Maryland Physical Therapist license.
• Strong assessment, documentation, and communication skills.
• Ability to lift/move up to 50 lbs and assist with resident transfers.
• Compassionate, reliable, and team-oriented.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for residents requiring physical therapy services.
• Deliver therapeutic exercises, balance training, mobility instruction, and pain management techniques to improve functional outcomes.
• Document all evaluations, progress notes, and discharge summaries accurately and in compliance with facility, state, and federal guidelines.
• Collaborate with the interdisciplinary care team—including occupational, speech, and restorative therapy to ensure coordinated resident care.
• Educate residents, caregivers, and families about safe mobility, transfers, and home exercise programs.
• Supervise and guide Physical Therapy Assistants (PTAs) and support staff as needed.
• Maintain equipment and ensure therapy areas are clean, organized, and safe.
• Participate in resident care meetings and provide input on progress and goal-setting.

The PT will evaluate, plan, and provide therapeutic interventions to residents in order to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
