Qualifications

• Strong communication, interpersonal, and time-management skills.

Responsibilities

• Conduct comprehensive initial evaluations to determine condition and rehab potential.
• Develop, implement, and modify individualized plans of care based on needs.
• Provide evidence-based PT treatments including strength, balance, and mobility training.
• Document progress and discharge plans per federal/state/facility guidelines.

Position Overview

We are seeking a compassionate, skilled, and motivated Physical Therapist (PT) to join our rehabilitation team full-time. In this role, you will evaluate, plan, and administer personalized physical therapy programs to help our residents achieve their maximum functional mobility. You will work closely with a multidisciplinary team of doctors, nurses, and fellow therapists to deliver top-tier post-acute care.

Key Responsibilities

• Conduct comprehensive initial evaluations of residents to determine their physical condition, limitations, and rehabilitation potential.
• Develop, implement, and modify individualized plans of care based on resident needs and physician orders.
• Provide high-quality evidence-based physical therapy treatments (including strength training, balance exercises, and mobility training).
• Accurately document resident progress, treatments, and discharge plans in compliance with federal, state, and facility guidelines.
• Educate residents and their families on post-discharge exercises, safety protocols, and assistive devices.
• Collaborate effectively with the interdisciplinary care team to optimize patient outcomes.

Qualifications & Requirements

• Education: Master’s or Doctorate degree in Physical Therapy from an accredited program.
• Licensure: Active, unrestricted Physical Therapist license in the State of California.
• Certification: Current CPR/BLS certification.
• Experience: Experience in a skilled nursing facility (SNF), post-acute care, or outpatient rehab setting is preferred, but passionate new graduates are welcome to apply!
• Strong communication, interpersonal, and time-management skills.
