ANGELCARE HOME HEALTH SERVICES LLC is seeking a compassionate Physical Therapist to join our Vallejo, CA team, delivering in-home care that improves mobility and quality of life. You will assess patients, create tailored treatment plans, and educate families in a home setting.

The role emphasizes collaboration with physicians and nurses, adherence to Medicare guidelines, and thorough clinical documentation. California licensure and CPR/BLS are required.

Qualifications

• Valid California Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Home health or outpatient rehabilitation experience preferred.
• Strong knowledge of Medicare home health guidelines and documentation requirements.
• Excellent communication, interpersonal, and patient education skills.
• Reliable transportation and valid driver’s license for home visit travel.
• CPR/BLS certification required or ability to obtain prior to start.

Responsibilities

• Evaluate patients' physical conditions, functional limitations, and rehabilitation needs through comprehensive assessments
• Develop and implement individualized physical therapy treatment plans to restore mobility and improve quality of life
• Provide therapeutic exercises, manual therapy, and other evidence-based PT interventions in a home health setting
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Educate patients and caregivers on home exercise programs, safety techniques, and injury prevention
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
