Job summary

CVHCare in California is hiring a Licensed Physical Therapist to provide home health services in designated Oakland/Berkeley areas. You will evaluate mobility, set goals and educate patients in the comfort of their homes, with a focus on safe mobility and independent progress.

Open to part-time schedules (32 hours/week or Per Diem) with benefits and a supportive team. Prior home health is welcome but not required; strong documentation and communication skills are essential.

Qualifications

• Licensed Physical Therapist in California with current credentials.
• BLS certification required or active CPR.
• Experience or openness to home health care environments.

Responsibilities

• Evaluate mobility, strength and function in patients served at home.
• Establish goals to improve balance, gait and daily activities.
• Assess home environment for safe mobility and adapt as needed.
• Educate patients on therapy use and equipment for safe progress.
• Set own schedule while maintaining compliance with agency guidelines.

Schedule: Part Time (32 hours p/week = 24 RVEs p/week) -comes with benefits!! or Part Time 24 = 18 RVEs p/week or Per Diem (6 visits p/week)

About the position:

Our Licensed Physical Therapists (PT) work in collaboration with the doctor, patient and the patient support system to provide best in class care for return to health and activity. As a Physical Therapist, you are responsible for the evaluation of mobility, strength and level of function. With this information you will establish goals to improve balance, walking, strength, as well as patient education. In the comfort of the patients’ home, the Physical Therapist assesses the home environment to ensure safe mobility is achieved. With your knowledge of best in class techniques, you will recommend equipment to enhance therapy, as well educate the patient on proper use.

In the unique setting of Home Health, each Physical Therapist sets their own schedule. You will find a great level of independence with the established support of an industry leader.

Requirements:

• Minimum one (1) year experience working as a Licensed Physical Therapist desired
• Open to entry level or candidates with no prior home health experience
• Current/active Physical Therapy License – State of California
• BLS Certification
• General knowledge and understanding of clinical verbiage
• Strong attention to detail for use in chart documentation
• Excellent time management/calendaring skills and ability to deliver documentation timely
• Strong experience using computers and computer systems
• Experience using Microsoft Suite (Word, Excel, Outlook), Gmail Suite or other systems
• Excellent written and verbal communication
