Sterling County Nursing Home is seeking a Physical Therapist PRN to join our team in Sterling City, Texas. The PT will develop and implement personalized plans of care for residents and short-term patients, while supervising staff PT Assistants and conducting screenings as needed.

Responsibilities

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.

• Assist as needed with patient screenings for the need for Physical Therapy.
• Communicate progress and any barriers toward PT goals with interdisciplinary team members.
• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow-up.

Job Summary:

Sterling County Nursing Home, a provider of short- and long-term nursing and rehabilitation services, is seeking a Physical Therapist PRN to join our team in Sterling City, Texas. As a Physical Therapist PRN, you will have the opportunity to develop and implement personalized Physical Therapy plans of care for facility residents and short-term patients, while providing supervision and training to staff Physical Therapy Assistants.
