Job summary

Matagorda Nursing & Rehabilitation Center is seeking a licensed Physical Therapist to assess patients, develop individualized rehabilitation plans, and deliver treatments in our SNF setting.

Work with the inter-disciplinary team, monitor patient progress, adjust plans as needed, and ensure thorough documentation in compliance with regulations. New grads are welcome; competitive compensation and flexible schedules are provided.

Qualifications

• Active/valid Texas license in Physical Therapy.
• In good standing with regulatory agencies and licensing boards.
• SNF/long-term care experience preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Consult with patients to learn about their physical condition.
• Develop and implement personalized rehabilitation programs.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Monitor patient progress and adjust plans with IDT collaboration.
• Document thoroughly and ensure compliance with regulations.
• Attend required meetings designated by the Director of Rehab.
• Collaborate with interdisciplinary team to ensure quality care.

Job description

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:
