Job summary

The Orchards at Southgate in Southgate, MI is seeking a PRN Physical Therapist to join its in-house therapy team. The role offers per diem scheduling with pay starting at $50.00 per hour, increasing with experience.

The facility provides short-term rehab and long-term skilled nursing care in a warm environment. As part of the rehab team, you will develop treatment plans, deliver therapy, and educate residents, families, and caregivers.

Qualifications

• Must have a Physical Therapist license and be able to plan and implement PT treatments.
• Ability to document patient progress and adapt plans as needed.
• Strong communication with patients, families, and staff.
• Collaborate with the in-house therapy team to coordinate care.

Responsibilities

• Plan treatment for residents and patients to relieve pain and improve function.
• Deliver treatments according to established care plans and state regulations.

DESCRIPTION

The Orchards at Southgate in Southgate, MI has an amazing opportunity for a PRN Physical Therapist( PT )to join their In-House Therapy Team. The Orchards at Southgate provides short-term rehabilitation and long-term skilled nursing care in a calm and relaxing environment. Our robust rehabilitation program challenges patients to meet and exceed expectations, with the overall goal of allowing them to return home faster and stronger than before.

Pay starts at $50.00 per hour and increases based on experience.

Essential Duties and Responsibilities:

• Plan treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Assure all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Monitor patient response to treatment intervention
• Complete required forms and documentation in accordance with company policy and state/federal regulations
• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized fitness-oriented health care programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain and improve mobility.
• Advising patients on exercise techniques.
• Advising patients and their families about in-home treatment options.
• Providing educational information about injury prevention, ergonomics and ways to promote physical health.
• Consulting and collaborating with other healthcare professionals.
• Documenting patient care history.
• Complying with rules, regulations, and procedures.
