Job summary

The Orchards at Samaritan in Detroit, MI is seeking a full-time Physical Therapist to join our in-house team serving residents with person-centered care and positive outcomes.

You will assess, treat, and develop rehab plans, collaborate with the interdisciplinary team, and ensure documentation meets regulatory requirements. New grads welcome; flexible schedules and competitive benefits offered.

Qualifications

• Must hold an active Michigan license in Physical Therapy.
• SNF/long-term care experience preferred.
• Knowledge of current treatment practices required.
• Familiarity with Net Health Electronic Documentation System preferred.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using various techniques.
• Creating personalized rehabilitation programs for patients.
• Administering prescribed therapies to relieve pain and restore function.
• Ensure all treatment aligns with the plan of care.
• Monitor patient response and modify plans with IDT collaboration.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Ensure timely and compliant documentation.
• Attend required rehab director meetings.

We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Pay starts at $40.00 per hour and increases based on experience.

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.

• Assessing and interpreting patient evaluations and test results.

• Developing treatment plans using a variety of treatment techniques.

• Creating personalized rehabilitation programs for patients.

• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

• Ensure all treatment delivered is in accordance with the established plan of care.

• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.

• Provide clinical support and education to patients, family members, and caregivers.

• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.

• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.

• Current knowledge of treatment practices

• Knowledge of Net Health Electronic Documentation System preferred.

• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Michigan license in Physical Therapy

• In good standing with all regulatory agencies and licensing boards
